What are house calls?

Our team provides primary care and chronic care management wherever you call home. This includes residential, assisted living, and skilled nursing environments. Our services are reserved for those with chronic conditions that have left them homebound. Our care is intended to help patients who no longer have the ability to leave their homes for doctor visits. These visits are performed in person or via telehealth. House Calls are billed similarly to a doctor’s visit. Copays may apply
